Requesting a Prescription for Speech Therapy

1. Call your child’s pediatrician and state that you are having concerns and would like a referral for speech language therapy evaluation.

2. Ask the doctor’s office to fax the prescription to (844-315-9608).

3. The prescription will need to have an order for a speech language evaluation. Diagnostic Code. Codes typically used for Speech Language Evaluation include:

• F80.0 Articulation/Phonological Disorder 

• R48.2 Apraxia

• F80.1 Expressive Language Disorder

• F80.2 Mixed Receptive-Expressive Language Disorder

• F80.81. Childhood-onset fluency disorder

• F80.89 Other developmental disorders of speech and language

• F80.4 Speech and Language Developmental Delay due to hearing loss
